Rainbow Pooch Pride Ltd collects the information requested in this form to support delivery of its activities made possible through donations and public funding. Your details help us ensure that activities are accessible, inclusive and appropriately designed to meet the needs of participants. I accept that there is a risk of transferal of infectious diseases, including kennel cough, involved in bringing my dog(s) to this event. I understand that vaccinations reduce but do not eliminate the risk; and will make no related claim against Rainbow Pooch Pride Ltd.
